CivicHealth can create a specialty or narrow network that supports internal communications within a post-acute care network (PACN) as well as communications with external agencies.
For example, PACNs use CivicHealth to notify a community partner of an incoming referral via secure text or email. The PACN can also give the community partner access to relevant patient information via a web-based portal enabled by CivicHealth. The community partner can then use CivicHealth technology for follow-up reporting to the PACN to confirm interactions with the patient and report progress.
CivicHealth’s community resource directory incorporates community and human services such as area agencies on aging, Meals on Wheels, transportation and religious organizations.
